Doctor Career

The Doctor career is one of the 3 active careers in The Sims 4 Get to Work Expansion Pack where you can follow your Sims to his/her workplace. Your Doctor Sim will work at the hospital but they also make house calls.

You will start as a Medical Intern but you can climb your way up to become the Chief of Staff. By leveling up in the Doctor career you will unlock clothing, objects and even a Trait: Sickness Resistance. What will you do as a Doctor? Flirt with your co-workers? deliver babies? or diagnose patients?

Inside the Hospital

The Hospital is only accessible when you need to work at the Hospital. Join your Sim at their workday or manage your Sims at home. When working in the Hospital you can find rooms for patients, analyzing, performing surgery, a room with X-Ray Machines and a room for Medical Treadmills. These are the ones you will be using.

Start your workday at the Hospital to diagnose patients, treat patients, perform surgeries, deliver babies and determine the gender of babies on pregnant Sim.

Improve your Career Performance

Start your workday by being Happy or better by being Focused. You can do this by browsing Simpedia on the Computer at home to gain a +1 Focused moodlet for 4 hours. If you are at the Hospital you can ‘Research Health Reports on Medical Journal’ on the computer to get a +2 Focused moodlet. You can also use cheats to gain a +2 Focused moodlet for 12 hours. The best way to improve career performance are:

  • Going to work with a positive emotion (Focused).
  • Completing the Daily Task.
  • Maintaining high relationships with Co-workers.
  • Learn and level up the Logic Skill to get a better result if you are analyzing samples on the Chemical Analyzer or the X-Ray Machine.
  • Learn and level up the Handiness Skill incase equipment breaks down. You can also use it to upgrade the Chemical Analyzer or the X-Ray Machine for better results.

Examine the patient to give them the right diagnose. Look for visible symptoms.

Start your day in the Hospital

First you will need to diagnose your patient that come in to the hospital. You can use the X-Ray Machine, run tests, or bring the patient to the examination table to check his eyes, ears and take his temperature. Another option is to put your patient on the Treadmill to diagnose them.

Tip: Stick with one patient while you are completing the tasks for your career goals instead of trying to get all the patients from the front of the desk to their bed and tests them.

Diagnose your patient from visible symptoms

Sims will show visible signs about their illness. This makes it easier for you to diagnose a patient. The list below will help you out to diagnose a patient from visible symptoms. Most of the symptoms can be lead back to more than 1 disease. The longer the patient stays in the hospital the more symptoms this Sim gets.

The Triple ThreatSurgery

After you diagnosed your patient they will need treatment to cure the disease. The patient who are ready for their treatment will have a red heart sign above their head. After the treatment the patient is ready to go home.

Deliver babies at the Hospital

Delivering babies at the Hospital can be done by your Doctor Sim when reaching level 8 in the Doctor Career. When pregnant Sims comes into the hospital and is going into labor you will have to bring them immediately to the Surgery Table to deliver the baby. When your Sims is level 6 in the Doctor career you can determine the sex of the baby before a pregnant Sim is going in to labor. This will also work on male Sim who have been impregnated by abduction.

Tip: You can influence the gender of your Sims offspring by eating Strawberries and listening to Pop music for a girl, and eat Carrots and listen to Alternative music for a baby boy. You only have more chance for that gender but it isn’t always certain.

Making House Calls

In your career as a Doctor you will sometimes get an ‘urgent’ notification that you need to make a house call. Making house calls is not a top priority, if you want you can skip it. When to wait to long the house call will vanish but it won’t effect your work performance in a negative way. If you’re accepting the house call just click on the front desk (the counter and not the computer) to travel to the right location. When you arrive on the location you will get a main goal assigned. You need to complete this goal to get a performance boost in your career. When you are done click on your Sim to go back to the Hospital.

House Calls
When put on a House Call you only have to treat the patients, you don’t have to diagnose them.

Doctor Career (Level Rewards)

When your Sims is reaching a higher level in the Doctor career you will be able to unlock career rewards such as; Diploma's, eye chart, and a Sim Skeleton Model. Doctor Sims can also unlock new interactions: give medical advise or give bad medical advise, new romantic interaction 'check for fever' and determine if pregnant Sim is going to have a baby boy or a baby girl.

Sickness Resistance TraitResilient immunity, Sims will have a lower chance of getting sick and will recover from sickness faster. The Sickness Resistance Trait is obtained by reaching level 8 in the Doctor career.

  1. This symptoms Dont work! Its so frustrating that my Sim can’t tell what is wrong and the symptoms I see are so unreliable!!!

    1. I’m working really hard on this list atm. and already invested 3 hours figuring this all out. I think there is one symptom for each disease that is unique and I’m trying to figure it out.

      1. Can someone tell me how you diagnose patients? Currently my sim is an R.N (or whatever that is) but for all i do tests and use the x-ray/treadmill, he/she still has to choose from 3/4 different diagnosis’ and unless your familiar with the symptoms (which is awkward to do) then I find myself completely misdiagnosing…. Good thing after the first few levels it doesnt mind if you fail anymore and you still get nearly $1000 a day (I used to fail and only get $22 lol)

        do you need to keep repeating tests? Or am I missing something obvious?

        1. What graphics settings are you playing on? I was playing on lower settings initially (because I’m on laptop) and I couldn’t for the life of me see any symptoms. Then I turned up the settings and immediately saw tiger stripes and green spots on my patients.

  2. My sim is an rn and I never see any opportunities to determine the gender of a baby. What do i have to click on??

    1. My Sim, Dylan, was able to tell his wife what gender their baby was, but the option didn’t become available until the second trimester. I think it was under friendly and was called “Determine Baby Gender,” but I might be wrong. Anyways, it’s just a friendly social, and I don’t think you use it for work.

    2. Yes, it is a friendly social interaction and it will have that little doctor symbol beside it. Alternatively, you could go through the special>doctor interaction menu and probably find it there. The only sims I’ve been able to determine gender of their baby are ones who are pretty “pregnant looking” lmao… I’d say probably 2nd – 3rd trimester only.

  3. When do pregnant Sims start coming to the hospital in labor? My Sim is level 7 in the career and I’ve yet to see any event regarding a Sim in labor. (Worst that happened so far was a Sim coming in and passing out in need of surgery)

    Also, a Sim came into the hospital glowing red, and when I did an x-ray scan on her it said several organs were out of place and that it was an anomaly. The machine wasn’t calibrated, but the work day cut off before I could calibrate it and try again. The wording used to describe it said that it might have been because of the lack of calibration, but it also said “best not to mention it until more tests have been conducted” or something along those lines. Does this mean there ARE anomalies? Especially since a glowing red Sim isn’t a listed symptom?

    (Sorry for really long comment lol)

    1. I had a Glowing red Sim while testing Sim for the symptoms list. I found out she was an Alien, so I didn’t put it down as a symptom but yes I can be wrong. And I was level 7 when my first pregnant Sim walked in. Be patient 🙂 they will come.

  4. I wish it would let us be at the hospital for longer. Like all the commercial stuff. I’m disappointed:/ it was not worth my money if won’t let me have more control.

    1. I totally second this! I hope someone brings out a mod that handles this for all the new careers. I also wish the retail shop was seen as a career for the sim created it. I know it won’t pratically work but I’d love for that to be the case regardless!

  7. I just had a sim come in that at one point had orange and green stripes, then the stripes went away to be replaced by a red glow. So frustrating…

  9. Since I’ve climbed up the career I realise that no other sim does anything with the patients except stand up and talk. Why is it my Sim has to do everything? This means while I’m trying to diagnose and treat patients there are those not even being showed to a bed and walking out the hospital. Why doesn’t the other staff in the hospital work!?!? Lol. Oh and why don’t I get any housecalls and I’m on Level 8?

  10. My sims first day at the hospital and her skills were too low to diagnose the patients. She could only take their temperature. But, there were no other doctors working. Just a nurse and receptionist. and they were useless. But it was still fun. my sim had a lousy day and was only paid $22 for the entire shift.

    I did have a different sim have a baby and that was fun to watch.

  12. Hi, I’ve been on career level 8 for about 11 sim days and a pregnant sim still hasn’t come into the hospital so I can’t complete the promotion requirements. Any tips?

    1. You could have one of your own sims get pregnant and do it that way or just use the doctor promotion cheat?. One small cheat can’t hurt right??

I’m at level 7 with my sim, but I keep getting the notice that the patient has left because I didn’t have time to treat, which doesn’t make sense cause I’m constantly trying to see if anyone approaches me with the diagnosis, but that never happens. Am I supposed to just know which patient I need to diagnose in order for the surgery to be on time or what? Really confused by this!

    1. If u re at level 7 of the doctor career then i suppose u can diagnose patients urself and then treat them. U need to run diagnostic tests from the patient interactions to come to a compelling or guaranteed diagnosis. Then u can treat the patient & when there is an empty bed available admit another patient ( from those who r waiting in the reception area ) to the exam bed and start with their diagnosis. Patients leave hospital because they r kept waiting for too long at the reception area. you have to be fast! 🙂 Good Luck.

I tried clicking on the dr and rhe option “pass over patient” (or whatever it’s called in the game, i don’t have it in English) but nothing happens….it disappears from the task thingy….is there anything special o have to do before i can pass over?

  17. What does the styled room “General Hospital” do exactly. I’ve unlocked it but I can’t use it as a business….or can you. I thought I might have been able to build that office in my home and work there seen as I’m level 10 in the doctors career. Is that an option?

      I already purchased the general hospital worth 7000 simoleons and placed it on my lot.
      Now how do i get the patients to come in my home clinic?

      1. I just looked it up somewhere else and it said : You can’t have your own private practice. Doctors can only practice medicine on the hospital lot.
        Sad! 🙁

  18. My sim is a level 8 and pretty much about to get promoted to level 9 and has yet to encounter a pregnant sim to deliver a baby :/

    1. You will have a lot more pregnant sims in your neighborhood if you use the mod “MC Command Center” it not only can modify many many things but can regulate stray and stick sims, ghosts etc You can modify the population settings so that women in relationships (or not in them) will get pregnant a lot more often. It’s a SUPER helpful mod. You can also enable child support, edit bill percentage, enable risky woohoo AND woohoo and pregnancy for teens. (I recommend a 3-5% risky woohoo rate with fertilization treatment setting OFF for the most “realistic experience”).

      As for this Doctor career….
      I find myself consistently getting diagnoses wrong when I am sure I am right! This only really started to happen a lot around level 7/8 of my career. Sometimes I will notice thought bubbles that aren’t usually associated (at least online) with the illness they have or “had”. I’m going to try increasing my logic skill a bit more and really pay attention to every little detail. Like someone else said I’ve seen someone who apparently had gas-and-giggles have a thermometer thought bubble when I was sure they didn’t experience fever symptoms >_<

      So yepp, this is a tricky one. I am going to do my best to help SimsOnline really flesh out this doctor guide, free of charge, naturally. 😉

  19. Does aging have to be turned on in order to get a pregnant sim into the hospital? I’m almost level 9 and I cannot complete the daily task to delivery a baby. Thank you!

  20. Another 2 symptoms of Sweaty Shivers are steam coming off of the entire body (and the sim is fanning themselves) and the thought bubble with a microwave on it.

  21. My game has a glitch. I can not do anything with the doctors career. I can’t seem to do the daily tasked. I have no bubble that says check ear, eyes, etc. help!!!

  22. Itchy Plumbob also get the tiger stripes. I have had 6 patient s now that have Itchy Plumbob and had the tiger stripes.

  23. I edited the hospital using enable free build then afterwards I can’t use the Surgery Table. Now I’m having problems when patient has an illness that needed to be treated by surgery. Especially when pregnant woman comes my sim can do nothing. Anyone having the same issue? 🙁

    1. I am having the same issues and i had to quit doctor career because of that. I cannot use the surgical table just because i edited the freaking hospital. Please if anyone has the same issue and know how to fix it help

    2. Hey I had this problem too. What you need to do is go into free build again, make SURE that you also apply the bb.showhiddenobjects cheat as well. There is usually (for some reason) about 6 “different” surgery tables that are exactly the same >_>. The annoying truth is only ONE of those 6 will work -_-. So I had a patient diagnosed with an illness that required surgical treatment and then I switched into build mode, I put down table #1 and table #2 (in order). And then tested the treat option to see if “treat with surgery” would un-grey, and it was. SO then I watched which one my doctor brought the sim to, remembered which number it was (for me it was #1), and replaced my second one with the #1 option in build, and now they both work! ^_^

      Ps. I was about ready to give up on the career too haha.

  24. My sim can’t use the surgery table even though my sim had the ability to use it. When I try it says it’s not a available or not able to be used??

    1. You need to g back into build/buy with bb.showhiddenobjects enabled and experiment with each surgery table (Switch in and out of build and live mode) until one of them allows that interaction again. Then you know that table is the “valid” table. I had this problem too.

  25. Please Help! I edited the hospital in the Sims 4 Get to Work with the bb.enablefreebuild cheat, so that it can look a bit more modern.Ended up that I can’t do any operations/deliver babies on the Surgery table. I then started a new game again and left the hospital as is and could carry out operations and deliver babies again .
    So I was being hopeful edit the hospital again hoping first time was just a glitch perhaps. And again I can’t do anything with the surgery table. Any other Simmers experiencing this or perhaps who had the same problem and know how to fix this?

    1. Maybe you could just try replacing the whole lot again from the gallery? Or find a a different hospital off the gallery. There’s this cool one by Deligracy I think called Private Hospital.

  26. Hey, I just noticed something new in patients with starry eyes. They’ll sometimes do this motion where it looks like they’re trying to swat at invisible flies. More often than not, patients that I saw doing this ended up having it.

  27. How do you make a hospital? I’m a doctor but I can’t go to work and I made a clinic but idk how to get sims to come or work there?

    1. You have to edit the actual hospital lot with bb.enablefreebuild & bb.showhiddenobjects (make sure you pay attention to the surgery tables, there are 6 listing of them in the catalog, and all of the information is the same BUT only one will be “valid” for interaction). I don’t remember right now if you can visit the hospital lot whenever you feel like it…. but I know for sure when your sim goes to the hospital to work THAT is when you can enable those cheats with ctrl + shift + c (you probably know that already xD) and re-design your hospital. Just make sure that you have at least 5 exam beds, and 1 of each machine (treadmill test, x-ray, surgery table) and don’t forget your front desk.

  28. I do not know if this is helpful or not. I have notice a box of tissues pop up when one of the sims has Llama Flu. I am going to play through the Doctor career a few times to get a better idea if each illness has a unique feature. My success rate is going well on this current playthrough, so I just might figure it out. Not all of the illnesses show all of their full symptoms in the early stages (such as the spots), so I can be hard to determine. But with Bloaty Head always seems to have an orange halo around their head. I know Carl Sims has a guide, but it’s far from perfect.

  29. People with Llama Flu will usually show thought bubbles that can construed as them having a high temperature, like the thermometer, along with the stuff listed above. I’ve personally witnessed it, so a lot of the symptoms are shared, with subtle difference differentiating all the illnesses. So basically, it doesn’t seem to be something you can quickly assess , and running tests can really help narrow down the possibilities.

    1. use the console command -> sims.modify_career_outfit_in_cas you ‘may’ need to have testingcheats true on if it doesn’t work. 🙂

  31. Reporting back with some helpful information! (I’ve been keeping a binder of notes in RL lmao)

    Sweaty Shivers and Burnin’ Belly can be mistaken easily for each other because they exhibit fairly similar (and common) symptoms.

    Sweaty Shivers:
    – Fever (Thermometer thought bubble.)
    – Itchiness (Bug thought bubble accompanied with sim scratching themselves.)
    – Splotchy Rash (Orangey-green spots)
    – Nausea (Bismoth thought bubble).

    Burnin’ Belly:
    – Fever (Thermometer thought bubble)
    – Nausea (Bismoth thought bubble)
    – No presence of rash

    Fever and Nausea symptoms tend to display most common, so the minute you see a: splotchy rash OR scratching you can immediately rule out Burnin’ Belly. If you’ve just arrived at the hospital, some sims who are already in the exam rooms will take a bit of time to develop another symptom, so it’s a good idea to narrow them down to what you can, and whilst keeping an eye on them, examine another sim. I had done this and while examining the other sim the previous sim developed a splotchy rash, and then I knew immediately it was Sweaty Shivers instead of Burnin’ Belly. (My 2 options at the time).

  32. I have been playing the doctor career path almost from the time I started the game. My sim has been stuck at level 8 for weeks now. When no pregnant sims came in I started making tons of sims pregnant. I have had the task for deliver baby come up three times now but when I look for the pregnant sim I can’t find one. I seriously have had my sims have 20 different pregnancies to help complete this task. I have edited her and her family in CAS to keep her young so I can get to the top of this career but no matter what I do she still hasn’t had a pregnant person come in during her shift. I am getting so frustrated and I just want to get her to the top of this career. Does anyone have any ideas why this is happening? Or how to fix it?
